GTN Technical Staffing Benefits Summary
GTN Technical Staffing offers an excellent benefit package to full-time employees who are scheduled to work 30 hours or more each week. GTN Technical Staffing offers a wide variety of benefits ranging from group sponsored plans to voluntary benefits. GTN Technical Staffing also offers different options and levels of coverage giving employees the opportunity to purchase the benefits that work for them. All of the health insurance benefits are effective on the first of the month following 30 days of employment. This document contains a brief summary of benefit options available to GTN Technical Staffing employees and is intended solely for the use of candidates considering an offer of employment from GTN Technical Staffing.

* These benefits will be effective on the first of the month following 30 days of employment and will be deducted during the previous month of coverage.

REFERRAL BONUSES
Ask your GTN recruiter or account manager about our referral program for consultants who stay with us for at least 90 days. This program is a benefit available exclusively to current and former GTN consultants.
HEALTH INSURANCE
You can select from two medical plan options through Blue Cross Blue Shield of Texas (http://www.BCBSTX.com). You can select from four levels of coverage; employee only, employee + children, employee + spouse, or employee + family. The primary differences between the two options are the premium amount, cost for office visits, annual deductible amount and annual out-of-pocket maximum. With both options, you may receive health care services from a list of network providers (PPO) or from any health care provider. When you use network providers (PPO), you pay a smaller percentage of the health care costs. The Health Savings Account (HSA) plan is a qualified high deductible health plan which allows you to open a health saving account to grow your money tax free while saving you substantial money on insurance premiums. The following chart generally describes the employee’s costs for each option which is effective September 1, 2011. The rates below reflect bi-weekly employee contributions.

Note that the insurance rates increased an average of 24% from 2009-2010 due to the mandates imposed by Obamacare after actually decreasing the previous year. These rate increases are only the beginning of what you will see unless Obamacare is repealed, as it is guaranteed to drastically increase health care costs and reduce the supply of doctors. You have a chance to restore the free market to health care insurance by voting in future elections for candidates who support limited government, fiscal responsibility, personal responsibility, rule of law, small government, and national sovereignty, all of which are principles that the Fabian socialists oppose.

A special note on Health Savings Accounts: When you enroll in a qualified high deductible health plan, you are covered for serious illness or injury by a high deductible (lower premium costs) insurance plan. You can then set up a health savings account (HSA) on your own. You can make tax-deductible contributions to an HSA to pay for qualifying medical expenses. The maximum amount per year you can put in this account is limited to the lower of the plan’s annual deductible amount or the statutory maximum as adjusted annually (up to $5,650 for a family in 2007). The best thing about a health savings account is that it can function like an IRA: you can put money in and let it grow tax deferred until you use it. You don’t have to use the HAS to pay for uncovered medical expenses, but if you do then you can withdraw the money tax free. GTN Technical Staffing recommends setting up an HSA with Sutton Bank (http://www.suttonbank.com/personal/checking/health-savings-account.html) because it allows you to transfer your money into a brokerage account with OptionsXpress to invest in a broader range of securities (most banks just pay low interest rates on HSA accounts).

SHORT TERM DISABILITY INSURANCE
If you elect Short Term Disability insurance through AIG, you can elect coverage in increments of $10 per week with a maximum benefit of $500 per week. Coverage is limited to a maximum of 60% of your weekly salary. Short Term Disability provides benefits for up to 13 weeks. The maximum weekly benefit for all levels of coverage is $500.

LONG TERM DISABILITY INSURANCE

ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE
Basic term life/AD&D is offered through AIG with a benefit of $15,000. This insurance is paid by GTN Technical Staffing, although you will see $1.52 of taxable income on your paycheck each month. Additional life insurance is not available due to lack of interest.

401(K) PLAN
After completing one year of employment, you can participate in the GTN Technical Staffing 401(k) plan through Fidelity Investments by contributing pre-tax dollars to the plan. GTN Technical Staffing provides a company match of 4% of your gross income for each pay period that contributions are made. You are automatically vested in all employee & employer contributions. The amount of the employee contribution can range from 1%-85% of annual compensation, to a maximum annual amount of $16,500. There is also a catch-up option available to those employees 50 years of age and older.

CONTINUING EDUCATION WITH NEW HORIZONS
While you are on assignment as a GTN Technical Staffing consultant you are automatically eligible for the following discounted training benefits from New Horizons.
Online Training New Horizons offers one of the best training sites on the Web! With just a few clicks, you can register online for more than 125,000 classes. Find exactly what you want, from the basics to technical training, in more than 170 locations in 26 countries. Once you visit their site you'll see why New Horizons is the world's largest and fastest growing computer training organization. Online courses are bundled into libraries of related skills that are available through one year licensed memberships. Once small fee covers all titles in the library you selected. An example of this would be the Microsoft Office Pack that includes all of the MS Office applications and all levels. This bundling allows you to take the training at your own pace and on your own schedule. You can take as few or as many of the courses in your library as you choose. As a GTN consultant you receive a 15% discount on each library you purchase, and once you complete at least one course in the library you can submit the course completion certificate to the accounting department at GTN for reimbursement of 65% of your investment in the course as long as you are still on assignment with us and have completed at least one month of service.
Classroom Training New Horizons can offer your classroom training at a 15% discounted rate. The normal rate for desktop application classes is $250 per day, and with your discount that would be $212.50 per day. On the high end technical level, such as project management, ITIL, Cisco, VMWare, and Microsoft server/back office, the rates range from $500 per day to $650 per day less the 15% discount. Reimbursement is not currently available for these classes.
